皮肌炎患者的年龄红细胞沉降率增快皮肤

Abstract:Theassociationbetweenidiopathicinflammatorymyopathy(IIM)andcancerhasbeenextensivelystudiedinadults.Manyepidemiologicalstudiesdemonstratedthisassociation,whichappearsstrongerfordermatomyositis(DM)thanforpolymyositis(PM).ThefirstcasesuggestinganassociationbetweencancerandDMwasreportedin.AtpresentthereportedincidenceofcancerassociationwithDMvarieswidely,fromlessthan7%toover30%.Manyearlyevidencescamefromcasereports,butthisassociationwaslaterconfirmedincase-controlaswellasinpopulationbasedstudies.OvariancancerorbreastcancerinfemalesandlungcancerinmalesarethemainmalignanciesassociatedwithDM.Giventhefrequencyoftheassociationofdermatomyositiswithcancer,forcost-effectivenessreasonsitmightbeimportanttodevelopsimpleandappropriatediagnostictestsforidentificationofpatientswithDM,whomaybeathigherriskofdevelopingamalignancy.Cliniciansshouldplanfollowupschedulestooptimizebothcancerdetectionandtreatment,andthustoimprovepatientsurvival.Manydifferentclinicalandserologicalsignshavebeensuggestedaspossiblepredictivefactorsformalignancyindermatomyositis:age,increasederythrocytesedimentationrate(ESR),presenceofcutaneousleukocytoclasticvasculitis,cutaneousrashandskinlesionsascutaneousnecrosisandperiunguealerythemas,neoplasticmarkersordysphagia.Theresultsofthedifferentstudiesarequitediscordant.Therefore,weconductedasystematicreviewofthescientificliteraturetoevaluatetheleveloftheriskofcancerinpatientswithdermatomyositisandtoexplorewhethercertainpatientcharacteristicsmaybelinkedtodifferentlevelsofcancerrisk.
